Job Description

Community & Family Resources (CFR) is an agency that assists individuals and families with substance abuse and gambling addictions and mental health needs.  CFR has an in-patient treatment facility and an in-patient adolescent treatment facility, both located in Fort Dodge. CFR also has outpatient offices located in Fort Dodge, Ames, Webster City, Boone, Clarion, Humboldt, Pocahontas and Rockwell City.  Our staff have the opportunity to provide support at many different levels with adults, as well as youth. 
We are looking to fill the position of Engagement Specialist/Case Manager.  This position case manages our clients to help ensure their compliance with their treatment needs.  This individual needs to be detail-oriented, individual and team contributor, and able to excel with minimal supervision. In addition, this individual must possess excellent verbal and written communication skills, have excellent listening comprehension, and this position requires a high degree of confidentiality. This employee works directly under the Mental Health Clinical Director, but will also perform duties for the Management Team. Clearly understanding the agency's missions, goals and direction as set by the Board of Directors is essential.

Skills & Experience

  • Bachelor's or Master's degree from an accredited educational institution, AND
  • At least three years' experience as a full-time case manager in a program similar to this agency is preferred.
  • Contacts clients referred by centralized scheduling who are not attending scheduled service appointments to confirm if they want services. The purpose of the contact is to: (1) identify barriers to client attendance and address them, (2) place clients into engagement groups or individual therapy to re-engage clients, and (3) if client cannot be re-engaged in treatment services, create discharge/transfer planning for client.
  • Screen and evaluate risks of consumers referred for services. Screen and evaluate requests for services.
  • Intervene, stabilize and manage acute crisis situations
  • Access and evaluate client needs, strengths, goals and resources
  • Refer the families/clients to the engagement/motivation group
  • Work with interdisciplinary team in reviewing cases and developing Individual Service Plans to re-engage clients into services
  • Provide linkage case management, participate in treatment planning and discharge planning when client is hospitalized
  • Consult with other CFR/TRC staff regarding assessments and service plan development
  • Consult with other service agencies around case-specific and programmatic services
  • Close charts for those clients who do not re-engage
  • Conduct follow-up satisfaction survey calls post-admission at 3, 6, and 12 months for 150 active and/or discharged clients annually
  • Distribute $25 gift cards to those who successfully complete the survey
  • Maintain 60% productivity
  • Other duties as assigned
