Job Description

Community & Family Resources (CFR) is an agency that assists individuals and families with substance abuse and gambling addictions and mental health needs.  CFR has an in-patient treatment facility and an in-patient adolescent treatment facility, both located in Fort Dodge. CFR also has outpatient offices located in Fort Dodge, Ames, Webster City, Boone, Clarion, Humboldt, Pocahontas and Rockwell City.  Our staff have the opportunity to provide support at many different levels with adults, as well as youth.
We are seeking a full-time Residential Treatment Intake Specialist for our Fort Dodge facility. This individual will coordinate the residential program referrals and admissions as intake process in accordance with agency policies and procedures as well as serving as a liaison to other agencies as assigned. The Intake Specialist will coordinate care of each client during admission with our financial, medical and clinical departments.
  1. Minimum two year degree in the social or behavioral sciences OR five years of experience in the social or behavioral sciences
  2. Experience working with persons experiencing co-occurring symptoms

Skills & Experience

  • Communicates with referring agency to determine if a potential client meets necessary requirements for residential
  • Conducts initial interviews and the treatment process with clients and referral sources
  • Provides clients/families with referral sources for services unavailable within the agency or when agency services are appropriate
  • Participates in intra-agency staffing to determine needed services and appropriate modes of treatment
  • All referrals will be staffed with the appropriate Residential Program Supervisor, clinical department, and medical department prior to scheduling admission appointment
  • Facilitates new client admissions and tour of facility with the admissions Treatment Specialist from the identified unit in which the client will be placed
  • Preadmission phone and/or in-person contact with client to discuss payment plans
  • Inform client about documentation to bring to first appointment
  • Completion of the PHQ9, DG SBIRT Screening and coordinating care with the appropriate department
  • Complete appropriate releases for appropriate client care
